Though not a direct answer to your questions, this post comes to mind:

Oooooh, now I get what you mean! You are talking about an issue tracking system (like JIRA or Mantis) where each issue has its own ticket with its own state and comments. I do absolutely support that! It doesn’t necessarily have to be in a state where everyone can register and create tickets (like it is e.g. with Bohemia Interactive’s “Feedback Tracker”, as they call it), but if everyone would have read-only rights, it would definitely be a big step towards transparency.

(where mantis and phabricator (which is what Bohemia Interactive use) are the open source options mentioned).
